Call for 2019 Award Nominations

Nominations can be submitted by any PSSNY member in good standing unless otherwise noted in specific award qualifications.

Please provide a letter of support for each award nominee describing detailed information about the candidate and why you feel they should be considered, include a current CV if possible.

All award nominations must be submitted to the PSSNY Awards Committee by March 15th, 2019.


Please fill out the following form with accurate information about the award you are nominating a person for and the information about the nominee.
 
DEADLINE FOR NOMINATIONS IS FEBRUARY 15TH!

Bowl of Hygeia Award:
  • The recipient must be a pharmacist, licensed in the state of New York;
  • The recipient must be living. Awards are not presented posthumously;
  • The recipient has not been a previous recipient of the award;
  • The recipient is not currently serving, nor has he/she served within the immediate past two years as an officer in other than an ex-officio capacity or its awards committee;
  • The recipient has compiled an outstanding record of community service, which apart from his/her specific identification as a pharmacist, reflects well on the profession;
  • The recipient shall be a member of PSSNY in good standing who has been a member for a minimum of four years.
Distinguished Young Pharmacist Award:
  • Licensed to practice pharmacy for nine years or less;
  • Licensed to practice pharmacy in New York State;
  • Member of PSSNY in good standing;
  • Participation in national pharmacy associations, professional programs, state association activities, and/or community service.
Excellence in Innovation Award:
  • Nominations are solicited from pharmacists in the State of New York;
  • The recipient must be licensed to practice in the State of New York;
  • The recipient must be a PSSNY member in good standing;
  • Recipient must be actively practicing pharmacy at the time of their nomination;
  • The nomination must include a complete description of the pharmacist's practice with an explanation of how they have improved patient care through rare, unique, and/or an innovative style of pharmacy practice resulting in improved patient care.
The Jack V. Nicolais Spirit of Pharmacy Award:
  • Presented to a member in good standing for at least 3 consecutive years; presenting a favorable impression of pharmacy to the public, performed service to the profession of pharmacy or pharmacy organization and aided in the preservation or advancement of community pharmacy.
